Fostering is a vital part of our rescue efforts, and we truly appreciate your willingness to open your home to a dog in need. Before we can find the best match for you and our rescue, we ask that you complete this application.
The first section includes our Foster Home Agreement, outlining important responsibilities and expectations. Following that, we’ll ask a few general questions to better understand your home, lifestyle, and experience.

Foster Home Agreement
1. Southern Comfort Animal Rescue (SoCo) agrees to allow name provided below to act as a foster home under SoCo Rescue's animal shelter license issued by the state of GA Department of Agriculture. The foster is hereby granted temporary possession of any animals that are placed as a foster animal in their home only.
2. No animal will be removed from or returned without authorization from a duly agent of SoCo Rescue.
3. Foster home will not accept or take possession of any stray or abandoned animals without authorization from a duly agent of SoCo Rescue while in the possession of the foster animal.
4. Foster home agrees to periodic inspections by SoCo Rescue personnel and/or The Department of Agriculture from the state of GA.
5. Foster home, the actual premises and all surrounding property, structures and all enclosures must be kept in good state of repair at all times and kept in a suitable condition for animals.
6. Each building will have a working fire extinguisher/smoke detector.
7. Temperature in indoor housing should be maintained at 55-85 degrees Fahrenheit.
8. All animals must have fresh water daily and checked during the day.
9. Medical treatment must be provided immediately when needed, please call Scott Bennett at (912)423-0145 to notify why medical treatment is necessary.
10. Foster shall only use SoCo Rescue approved practitioners for any and all medical care/treatment that may occur during the foster period. Foster must have pre-approval to use an outside facility. In case of emergency and pre-approval is impractical, then Foster, as soon as circumstances permit, must notify SoCo Rescue of the situation. Once the animal is stabilized, Foster must have any and all medical records sent to SoCo's veterinary facility at: East Windsor Animal Hospital, Dr. Rashid, 614 Rte 33 E, East Windsor, NJ, 08520 (609) 443-7500
11. No foster animal shall be held by means of tethering, chained or being housed outdoors. Fostering is to help our animals adjust for their forever homes.
12. SoCo Rescue will provide vaccinations, wormings and food for the foster dog(s).
13. Reimbursement of goods and/or services purchased for the foster dog(s) must be pre- approved by SoCo Rescue.
RELEASE OF LIABILITY, WAIVER OF CLAIMS AND INDEMNITY AGREEMENT
I freely accept and fully assume all such risks, damages and hazards, including the possibility of personal injury, death, property damage or loss resulting there from.
In consideration of my volunteering with SoCo Rescue and with the understanding the inherent risk working with Rescued Animals, I agree as follows:
To Release SoCo Rescue, it's officers, directors, shareholders and agents from any and all liability for any loss, damage, injury or expense that I may suffer caused by the Rescue Animal due to whatsoever INCLUDING NEGLIGENCE, BREACH OF CONTRACT OR BREACH OF ANY STATUTORY OR OTHER DUTY OF CARE.
In entering into this agreement, I am not relying upon any oral or written representations or statements made by SoCo other than what is set forth in this agreement.
